Not being able to consume citizens’ blood means you will be very underleveled but luckily the game is relatively easy and you can restore health with the Autophagy skill when needed. However, avoid turning any districts hostile (health below 50%), this would kill everyone in it and then you can no longer get the collectibles from those citizens. If citizens turn into monsters or go missing that’s okay and even killing them in monster form does not void “Not even once”. Always spare them or sacrifice XP to turn them into vampires. There are also a few story-related characters linked to trophies whose fate you must decide. So don’t bother running all over the map to heal people, they would just get sick again but no sickness can ever kill them. They can never die from sickness no matter how long you wait. The trophy concerns only non-hostile named citizens (the people from citizen menu). Killing enemies (human vampire hunters who fight you in the streets) is okay and does not impact the trophy.
